Boarding pets must be current on required vaccinations and free of fleas and ticks. If fleas or ticks are found during admission or during your pet's stay, they will be treated at the owner's expense.

If my pet is on a prescription diet, I agree to assume the cost of the diet(s) that are dispensed while boarding.

If medications are necessary for treatment or handling, I give my permission to Cryan Veterinary Hospital to administer such medications. I understand that medication administration fees may apply.
